Closing the loop: phase-dependent modulation of cortico-basal ganglia circuits.

Abstract

Closed-loop neuromodulation, whereby features of ongoing brain signals are used to control the timing of electrical or optical stimulation, has the potential to provide precise methods for the manipulation of functional and dysfunctional neuronal activity. Neuronal oscillations provide an attractive input signal for such closed-loop modulation, as they provide a robust signal to inform the timing of stimulation. As oscillations are a fundamental property of normal brain activity, modulating their strength and manipulating synchronization will likely influence brain function and ongoing behavior. Computational studies suggest pulsatile perturbations timed to a specific phase of an oscillation can modulate neural oscillators by shifting the relative timing of neuronal activity. Using a combination of preclinical and clinical physiology and bioengineering experiments, we have developed a viable system for utilizing such phase-dependent closed-loop stimulation to modulate pathological beta-frequency oscillations in Parkinson’s disease. In addition to providing a novel therapeutic strategy, these methods could allow the manipulation of a wide range of oscillatory activities to causally examine the role of rhythmicity in the healthy brain.
